class=\"wp-block-list\">\n<li><strong>Definition<\/strong>: Klinefelter syndrome is the most common genetic cause of <strong>primary hypogonadism<\/strong> in phenotypic males.<\/li>\n\n\n\n<li><strong>Incidence<\/strong>: ~1 in 1000 live-born phenotypic male births.<\/li>\n\n\n\n<li><strong>Typical Karyotypes<\/strong>:\n<ul class=\"wp-block-list\">\n<li><strong>47,XXY<\/strong> (most common)<\/li>\n\n\n\n<li>Variants: <strong>48,XXXY<\/strong>, mosaicism (46,XY\/47,XXY), or even 46,XX (phenotypic male).<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Genetic Mechanism<\/strong>:\n<ul class=\"wp-block-list\">\n<li><strong>47,XXY<\/strong> usually results from nondisjunction of sex chromosomes during parental gametogenesis.<\/li>\n\n\n\n<li><strong>Mosaicism<\/strong> arises from mitotic errors after fertilization.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-embed is-type-video class=\"wp-block-list\">\n<li>Becomes apparent at\/after <strong>adolescence<\/strong>.<\/li>\n\n\n\n<li><strong>Small, firm testes<\/strong> with <strong>hyalinized seminiferous tubules<\/strong>; disrupted Leydig cell function.<\/li>\n\n\n\n<li><strong>Azoospermia<\/strong>, testicular failure \u2192 decreased testosterone, infertility.<\/li>\n\n\n\n<li>May have a <strong>small phallus<\/strong>, gynecomastia, and some degree of eunuchoid proportions (due to delayed epiphyseal fusion).<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Hormonal Changes<\/strong>\n<ul class=\"wp-block-list\">\n<li><strong>\u2191 LH and \u2191 FSH<\/strong> (due to primary testicular failure).<\/li>\n\n\n\n<li><strong>\u2193 Testosterone<\/strong>, variable increases in estradiol \u2192 risk of gynecomastia.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Spectrum of Masculinization<\/strong>\n<ul class=\"wp-block-list\">\n<li>Ranges from moderate eunuchoidism to near-normal male phenotype.<\/li>\n\n\n\n<li><strong>Gynecomastia<\/strong> severity is highly variable.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Histology<\/strong>\n<ul class=\"wp-block-list\">\n<li>Seminiferous tubules show thickened basement membranes, sclerosis, and hyalin deposits.<\/li>\n\n\n\n<li>Leydig cells may be clumped or increased in number; can form adenoma-like nests.<\/li>\n\n\n\n<li>Before adolescence, testes may appear near-normal pathologically.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Other Clinical Issues<\/strong>\n<ul class=\"wp-block-list\">\n<li>Typically <strong>no multiple congenital anomalies<\/strong> (unlike Turner syndrome).<\/li>\n\n\n\n<li>Mild mental impairment or learning disabilities (especially verbal) can occur.<\/li>\n\n\n\n<li>Increased psychosocial and behavioral difficulties (anxiety, depression, poor judgment).<\/li>\n\n\n\n<li>Higher risk for <strong>pulmonary disorders<\/strong> (emphysema, bronchiectasis), certain <strong>cancers<\/strong> (mediastinal germ cell tumors, breast cancer), <strong><a href=\"https:\/\/myendoconsult.com\/learn\/diabetes-mellitus\/\" data-wpil-monitor-id=\"208\">diabetes mellitus<\/a><\/strong>, and <strong>varicose veins<\/strong>.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>46,XX Male Variant<\/strong>\n<ul class=\"wp-block-list\">\n<li>Similar phenotype to Klinefelter but often with <strong>hypospadias<\/strong> and <strong>shorter stature<\/strong>.<\/li>\n\n\n\n<li>Arises from <strong>translocation of SRY<\/strong> to the X chromosome.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>47,XYY<\/strong> Karyotype**\n<ul class=\"wp-block-list\">\n<li>Phenotypically normal males, <strong>usually fertile<\/strong>, typically normal testicular function.<\/li>\n\n\n\n<li>May have increased risk of speech\/language developmental delays and learning disabilities.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Diagnosis<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Clinical Suspicion<\/strong>\n<ul class=\"wp-block-list\">\n<li><strong>Tall adolescent\/adult male<\/strong> with <strong>small firm testicles<\/strong>, <strong>gynecomastia<\/strong>, <strong>infertility<\/strong>.<\/li>\n\n\n\n<li>Often presents with delayed or incomplete puberty.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Laboratory Findings<\/strong>\n<ul class=\"wp-block-list\">\n<li><strong>Karyotype<\/strong> of peripheral leukocytes confirms extra X material (e.g., 47,XXY).<\/li>\n\n\n\n<li><strong>Serum LH, FSH<\/strong>: Elevated.<\/li>\n\n\n\n<li><strong>Testosterone<\/strong>: Low or low-normal.<\/li>\n\n\n\n<li><strong>Inhibin-B<\/strong>: Decreased.<\/li>\n\n\n\n<li><strong>Semen analysis<\/strong>: Typically azoospermia.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Management<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/myendoconsult.com\/learn\/oral-trt-testosterone-replacement-therapy\/\" data-wpil-monitor-id=\"210\">Testosterone Replacement<\/a><\/strong>\n<ul class=\"wp-block-list\">\n<li>Addresses hypogonadism: improves virilization, muscle mass, bone density.<\/li>\n\n\n\n<li>Dosage titrated to maintain mid-normal <a href=\"https:\/\/myendoconsult.com\/learn\/clomid-or-testosterone-injections-in-male-hypogonadism\/\" data-wpil-monitor-id=\"209\">male testosterone<\/a> levels.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Fertility Considerations<\/strong>\n<ul class=\"wp-block-list\">\n<li>Most men with Klinefelter syndrome are infertile (azoospermia).<\/li>\n\n\n\n<li>Rarely, mosaic patients (46,XY\/47,XXY) may have some spermatogenesis.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Counseling and Support<\/strong>\n<ul class=\"wp-block-list\">\n<li>Address learning disabilities, psychosocial issues, risk of breast cancer and other comorbidities.<\/li>\n\n\n\n<li>Genetic counseling recommended, especially for variant
