data-wpil-monitor-id=\"288\">diabetes mellitus<\/a><\/strong><\/li>\n\n\n\n<li><strong>Dyslipidemia<\/strong><\/li>\n\n\n\n<li><strong>Atherosclerotic vascular disease<\/strong><\/li>\n\n\n\n<li><strong>Hypertension<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Key Characteristic<\/strong>: Most individuals are overweight (BMI 25\u201329 kg\/m^2) or obese (BMI \u226530 kg\/m^2), with <strong>excess abdominal visceral fat<\/strong>.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">PREVALENCE AND SIGNIFICANCE<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Approximately <strong>50 million<\/strong> people in the United States are estimated to have the metabolic syndrome (based on ATP III guidelines).<\/li>\n\n\n\n<li>Individuals with this syndrome have a <strong>2- to 4-fold<\/strong> increased risk of subsequent <strong>cardiovascular events<\/strong>.<\/li>\n\n\n\n<li>Debate exists on whether the metabolic syndrome is a unique entity or if it simply reflects the combined effects of its components.<\/li>\n\n\n\n<li>Regardless, <strong>identifying and managing these risk factors<\/strong> is crucial to lowering cardiovascular disease and diabetes-related morbidity and mortality.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">PATHOPHYSIOLOGY<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Insulin Resistance<\/strong>:\n<ul class=\"wp-block-list\">\n<li>More than normal amounts of insulin needed for a normal biologic response.<\/li>\n\n\n\n<li>Inferred by <strong>high fasting insulin levels<\/strong>.<\/li>\n\n\n\n<li>Affects muscle, liver, and adipose tissues \u2192 leads to <strong>decreased peripheral glucose and fatty acid use<\/strong>.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Prothrombotic and Proinflammatory State<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Increased levels of <strong>C-reactive protein (CRP)<\/strong>, <strong>plasminogen activator inhibitor 1 (PAI-1)<\/strong>, <strong>interleukin 6<\/strong>, and <strong>adipocyte cytokines<\/strong> (e.g., adiponectin).<\/li>\n\n\n\n<li>Supports the idea that the metabolic syndrome is not just about insulin resistance but also involves <strong>inflammation<\/strong>.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">DIAGNOSTIC CRITERIA<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">ATP III Criteria (Adult Treatment Panel III)<\/h3>\n\n\n\n<p><strong>Any three<\/strong> of the following <strong>five<\/strong> traits:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Abdominal obesity<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Waist circumference >102 cm (men) or >88 cm (women)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Serum triglycerides<\/strong>:\n<ul class=\"wp-block-list\">\n<li>\u2265150 mg\/dL or on medication therapy for <a href=\"https:\/\/myendoconsult.com\/learn\/topics\/hypertriglyceridemia\/\"  data-wpil-monitor-id=\"289\">hypertriglyceridemia<\/a><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>HDL cholesterol<\/strong>:\n<ul class=\"wp-block-list\">\n<li>&lt;40 mg\/dL (men) or &lt;50 mg\/dL (women), or on medication therapy for low HDL<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Blood pressure<\/strong>:\n<ul class=\"wp-block-list\">\n<li>\u2265130\/80 mm Hg or on medication for hypertension<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Fasting plasma glucose<\/strong>:\n<ul class=\"wp-block-list\">\n<li>\u2265100 mg\/dL or on medication therapy for hyperglycemia<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\">World Health Organization (WHO) Criteria<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Insulin resistance<\/strong> (<a href=\"https:\/\/myendoconsult.com\/learn\/topics\/type-2-diabetes\/\"  data-wpil-monitor-id=\"290\">type 2 diabetes<\/a>, impaired fasting glucose, or impaired glucose albumin excretion \u226520 \u03bcg\/min) or albumin-to-creatinine ratio \u226530 mg\/g<\/li>\n<\/ol>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">CLINICAL EVALUATION<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Patients with one or more risk factors<\/strong> for the syndrome should undergo:\n<ul class=\"wp-block-list\">\n<li>Thorough history<\/li>\n\n\n\n<li>Physical exam (including <strong>blood pressure<\/strong>, <strong>BMI<\/strong>, <strong>waist circumference<\/strong>)<\/li>\n\n\n\n<li><strong>Fasting lipid profile<\/strong><\/li>\n\n\n\n<li><strong>Fasting plasma glucose<\/strong> measurement<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">MANAGEMENT<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Lifestyle Modifications<\/strong> (cornerstone of therapy):\n<ul class=\"wp-block-list\">\n<li><strong>Weight reduction<\/strong> and increased <strong>physical activity<\/strong><\/li>\n\n\n\n<li><a href=\"https:\/\/myendoconsult.com\/learn\/low-carbohydrate-diets-for-managing-diabetes\/\"  data-wpil-monitor-id=\"291\">Diet and exercise can <strong>prevent\/delay onset of diabetes<\/strong><\/a> in those with impaired glucose tolerance.<\/li>\n\n\n\n<li>Aim for \u226530 minutes of <strong>moderate-intensity physical activity<\/strong> daily.<\/li>\n\n\n\n<li><strong>Smoking cessation<\/strong> is strongly advised.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Medical Therapies<\/strong>:\n<ul class=\"wp-block-list\">\n<li>In patients with <strong>impaired fasting glucose<\/strong> or <strong>type 2 diabetes<\/strong>, <strong>metformin<\/strong> can improve glycemic control by enhancing insulin action.<\/li>\n\n\n\n<li>Manage hypertension, dyslipidemia, and hyperglycemia per standard clinical guidelines.<\/li>\n\n\n\n<li>Because the metabolic syndrome is a <strong>coronary risk equivalent<\/strong>, LDL targets should be those recommended for high-risk patients (see standard LDL goals).<\/li>\n\n\n\n<li>If <strong>Framingham risk<\/strong> is >10%, consider <strong>low-dose aspirin (81 mg\/d)<\/strong> to reduce cardiovascular
