The three core pathophysiologic perturbations were first highlighted by deFronzo in the triumvirate, and this has since evolved into the <a href=\"https:\/\/myendoconsult.com\/learn\/the-ominous-octet-of-t2dm-m-o-a\/\">ominous octet<\/a> (8 defects) and, more recently, <a href=\"https:\/\/myendoconsult.com\/learn\/egregious-eleven-of-type-2-diabetes-mellitus\/\">egregious eleven <\/a>(11 defects). This represents are significant deviation from a insulin-centric approach to managing diabetes mellitus. A complete list of infographics is available at the end of this article.<\/p>\n<h2>Sensitizing<\/h2>\n<p><b>Metformin,<\/b> <b>Pioglitazone<\/b>, and <b>Dopaminergic agonist<\/b> (bromocriptine)<\/p>\n<p>Metformin by activating the AMP-kinase pathway, reduces hepatic glucose production. It also increases insulin sensitivity(1).<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/2-min.jpg\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" srcset=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/2-min.jpg 1024w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/2-min-300x225.jpg 300w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/2-min-768x576.jpg 768w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/2-min-624x468.jpg 624w\" alt=\"\" width=\"1024\" height=\"768\" \/><\/p>\n<ul>\n<li>Pioglitazone activates nuclear transcription factor PPAR-gamma and improves insulin sensitivity.<\/li>\n<li>Dopaminergic agonists (bromocriptine) improve insulin sensitivity.<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/6-min.jpg\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" srcset=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/6-min.jpg 1024w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/6-min-300x225.jpg 300w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/6-min-768x576.jpg 768w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/6-min-624x468.jpg 624w\" alt=\"\" width=\"1024\" height=\"768\" \/><\/p>\n<h2>Squeezing<\/h2>\n<p><b>Sulfonylureas <\/b>and <b>Meglitinides<\/b> (directly increase insulin secretion), <b>GLP-1<\/b>, <b><a href=\"https:\/\/myendoconsult.com\/learn\/tirzepatide-twincretin\/\">Dual GLP-1<\/a><\/b> and <b>DPP-4 inhibitors<\/b> (<a href=\"https:\/\/myendoconsult.com\/learn\/what-is-the-incretin-effect\/\">incretin pathway<\/a>), and <b>Bile Acid sequestrants<\/b> (increase incretin levels).<\/p>\n<p><strong>Sulfonylureas <\/strong>(glyburide, glipizide and glimepiride) and <strong>meglitinides<\/strong> (repaglinide and nateglinide) close potassium-ATP channels on the plasma membrane of beta cells, which induces the secretion of insulin (review the mechanism here).<\/p>\n<p><strong>Incretin mimetics <\/strong>(<a href=\"https:\/\/myendoconsult.com\/learn\/mechanism-of-action-of-glp-1-agonists\/\">GLP-1 and dual GIP-GLP1 agonists) activate GLP-1 receptors and produce the following effects<\/a><\/p>\n<ul>\n<li>Glucose-dependent insulin secretion (insulin is released only after meal intake)<\/li>\n<li>Reduction in glucagon release (after meal intake)<\/li>\n<li>Reduces gastric emptying, reducing a rise in blood sugar after a meal.<\/li>\n<li>Increased satiety and weight loss<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/4-min.jpg\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" srcset=\"https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/4-min.jpg 1024w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/4-min-300x225.jpg 300w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/4-min-768x576.jpg 768w, https:\/\/endoconsult.b-cdn.net\/wp-content\/uploads\/4-min-624x468.jpg 624w\" alt=\"\" width=\"1024\" height=\"768\" \/><\/p>\n<p><a href=\"https:\/\/myendoconsult.com\/learn\/dpp4-inhibitor-mechanism-of-action\/\"><strong>DPP4 inhibitors<\/strong><\/a> (sitagliptin, alogliptin, linagliptin, saxagliptin) inhibit the activity of dipeptidyl peptidase, thus increasing postprandial concentrations of incretins (GIP and GLP-1). These agents also reduce the release of glucagon.<\/p>\n<p><em>The proposed mechanism of action of bile acid sequestrants (colesevelam)<\/em><\/p>\n<ul>\n<li>Binds bile acids in the intestine and increases hepatic bile acid release. This ultimately reduces hepatic glucose production.<\/li>\n<li>Increases incretin levels<\/li>\n<\/ul>\n<h2>Flushing<\/h2>\n<p><strong>Alpha-glucosidase inhibitors<\/strong> and <strong><a href=\"https:\/\/myendoconsult.com\/learn\/mechanism-of-action-of-sglt-2-inhibitors\/\">SGLT-2<\/a> inhibitors<\/strong>.<\/p>\n<p>Flushing of glucose via the urinary system (peeing) or the gastrointestinal system (pooing). Intestinal alpha-glucosidase is required for the breakdown of gastrointestinal carbohydrates. Inhibitors of this enzyme include <strong>acarbose <\/strong>and <strong>miglitol<\/strong>.<\/p>\n<p><strong>Sodium-Glucose Transporter<\/strong><a href=\"https:\/\/myendoconsult.com\/learn\/glucose-transporters\/\"> 2 channels in the <strong>proximal nephron<\/strong> are required for glucose<\/a> reabsorption.
